These forms were created by the snakebite hub members to aid those pursuing clinical research on people affected by snakebites. These forms can be used for both primary data collection, and for standardising secondary data that has already been collected. We recommend using the Word format if collecting primary data and Excel format if recording secondary data. Further, there’s a full version and “minimum” version of the data collection forms. The minimum version was voted on by snakebite hub members based on what was thought to be both important and feasible to collect. Currently, the minimum form is being piloted in diverse settings. We will further refine it based on this pilot, and a final version will be posted on the hub when ready.
